Eisenhower Middle School Guidance and Health
On behalf of the Counseling Department and the Nurse's Office, welcome!
The Guidance program is an essential tool for the success of students at the middle school level. Through our program, we strive to help students not only make the right academic decisions, but we also attempt to guide them as they go through adolescence. Teenagers experience the most change during these crucial years, and we are committed to aiding them socially, emotionally, and cognitively.
The Nurse's office aids in providing medical care to the student body of EMS, as well as, valuable resources to both parents and students including various medical and athletic forms required by Roxbury schools.

Spring Is Here & So Are the Spring Sports!
Forms can be downloaded from the EMS Forms page on the web site, or obtained from the EMS health office.
Your child’s physical must be within 365 days of the start of Sports try-outs, practice or events. If your child’s physical is current, has participated in a fall sport, and on file in the health office, you will only need to submit the emergency contact/permission form and an updated parent questionnaire.
Physical Education Excuses
If your child is not feeling well enough to participate in gym class for one day or several days, and no doctor’s note is provided, a gym note from the parent is needed.
Without the parent note, students need to change into their gym clothes, or the student is marked as unprepared.
To avoid this, send a note in to excuse your child from gym, and changing into gym clothes will not be necessary.
