ROXBURY HIGH SCHOOL ANNOUNCES

SPORTS/BAND PHYSICALS PROCEDURE

Roxbury High School has announced its procedures for Sports/Band Physicals for Fall 2003. Students may receive physicals either from their own health care provider or from the school physician, by appointment only. Appointments will be on Wednesday, July 23, 2003, from 7:30AM to 4:00PM and on Wednesday, August 6, 2003, from 7:30AM to 4:00PM and will be scheduled on a first-come first-served basis.

Appointments for physicals can be made by calling the Roxbury High School Health Office by June 24, 2003 at 973-584-1200, ext. 258 or 259, between the hours of 7:30AM and 2:30PM. After June 24, 2003, students should call the Athletic Department at 973-584-1200, ext. 230 to schedule a physical. Students who miss their appointments will need to make alternate arrangements.

Only completed forms will be accepted in the Health Office. In order to ensure that student medical forms are complete and in compliance with state regulations, all students who have their physicals performed at their medical home must submit their forms to the Health Office. Forms will be accepted on any of the following days: from 8:00AM to 12:00PM on July 22, 2003, July 29, 2003, August 5, 2003, August 12, 2003; from 7:00AM to 4:00PM on July 23, 2003 and August 6, 2003; and from 8:00AM to 3:00PM on August 18 and 19, 2003. Completed forms may be dropped off in the main office of the high school on other days, however, they will not be reviewed for completion and accuracy until a later date. Students who submit forms after these dates or incomplete will not be cleared to practice for fall sports or band.

Students who have a history of Asthma or a life-threatening allergy must submit additional forms. These forms must be signed by both the medical home and the parent/guardian. Forms can be obtained from the high school Health Office.
