Attention Sixth Grade Parents/Guardians-Mandated Immunization Requirements for Sixth Grade

You were notified of changes in Chapter 14 of the New Jersey State Sanitary code “Immunization of Pupils in Schools” on April 15, 2010. This change involves students entering 6th grade in September 2010.  In order to process sixth grade placement letters in a timely fashion, we asked that all immunization documentation be received by the Lincoln Roosevelt Health Office by August 15, 2010.  As you know this will be a time consuming job.  Your cooperation is greatly appreciated.

We have several students whose 11th birthday does not fall until after August 15, 2010.  The Menactra (Meningitis) and the Adacel (Tdap) vaccines cannot be given until these children have reached their 11th birthday as per the pharmaceutical guidelines. Boostrix (Tdap) can be given after the 10th birthday.  If your child is one of these students he or she must submit a note from the doctor with his or her intended appointment date for receiving the immunizations on a doctor’s prescription or doctor’s letterhead.  This will allow your child to receive the 6th grade placement letter and start school on a provisional basis.  If your child does not receive these immunizations, he or she will be excluded from school until the shots are completed. We must comply with the new state mandates.

The new regulations state that if a student has had a DPT, DTaP, or Td within the last five years he or she does not need a booster at this time.  However, we need documentation of the date the shot was administered on a doctor’s prescription or letterhead. The records in school may not be up to date if boosters were given and proper documentation was not sent to school to up date the files.

Again, please remember time is running out.  Placement letters will NOT be mailed to addresses that have not complied with this state mandated requirement.

The documentation may be mailed, faxed (973-584-4257), or dropped off in the Main Office of Lincoln Roosevelt.

I apologize for any inconvenience.  Due to the rise in the occurrence of Meningitis, Tetanus and Pertussis within our population. it is very important for your child’s well being that he or she receives these immunizations. Our goal, as part of the health/education team, is to keep your children healthy and in a safe environment.
