PRESCHOOL           SCHOOL AGED

Parent Check List for students of Preschool Disabled becoming 5 years of age by June 30th.

 

Reevaluation Meeting 

________By June 30 of a student’s last year of eligibility for a program for preschoolers with disabilities a revaluation shall be conducted.
                  The parent is member of this Team.
 

_______All evaluations require Parental consent. 

_______The parent must receive copies of all evaluation 10 days prior to eligibility/classification meeting. 

 

Classification Meeting 

________At this meeting evaluations will be reviewed to decide if your child is eligible for special education and related services under one or
                  more of the categories in N.J.A.C.6A:14:2.3(i)1. 

 

IEP Meeting 

________If your child is found to be eligible for special education and related services, this meeting may be held the same day or at a later
                  date.  The IEP Team of which the parent is a member will develop and Individual Educational Program. 

________A new IEP is written annually or sooner if the child needs a change of placement.

Parent Check List

 

Referral 

_______Submit a written request for an initial evaluation to determine if child is eligible for services under N.J.A.C.6A:14. 

_______District has 20 calendar days excluding school holidays but not summer vacation to hold an identification meeting.  The purpose of this
                meeting is making a decision whether to evaluate. 

Identification Meeting 

______A meeting to review student records in order to make a decision to evaluate or not evaluate you child by a multi-disciplinary team. The
              parent is a member of this Team.

 

Evaluation Meeting 

_______If at the Identification Meeting it was decided to evaluate your child, then an evaluation plan must be made by the IEP Team to
               determine if you child is eligible for Special Education and Related services.  

_______All evaluations require Parental consent. 

_______ The District has 90 days to complete the evaluations and hold an Eligibility Meeting.   The parent must have copies of all evaluation
                   reports 10 days prior to the meeting.

 

Classification Meeting 

________At this meeting evaluations will be reviewed to decide if your child is eligible for special education and related services under one or
                  more of the categories in N.J.A.C.6A:14:2.3(i)1.

________If your child is found to be eligible for special education and related services, this meeting may be held the same day or at a later
                  date.  The IEP Team of which the parent is a member will develop and Individual Educational Program.

Reevaluation Meeting 

________Every three years or sooner if conditions warrant a reevaluation is conducted to determine if continues to be a child with a
                  disability.  The parent is member of this Team.
